What you will do:

- Conduct person-centred, strengths-based assessments and develop care plans in accordance with the Care Act 2014.
- Facilitate safe and timely discharges from hospital settings by utilizing the Home First and Discharge to Assess model.
- Design and coordinate tailored care packages that foster independence and encourage recovery.
- Manage a dynamic caseload involving adults with complex needs in a rapidly evolving environment.
- Work collaboratively with NHS staff, families, and partner agencies to ensure a holistic approach to care.
- Recognize and address safeguarding issues by aligning with statutory requirements.
- Maintain comprehensive and up-to-date records, documenting reports and regularly reviewing care plans to ensure quality and compliance.
