What you will do:

Conduct assessments and reviews of children and young people with disabilities to ensure their needs are being met effectively.
Collaborate with families, healthcare professionals, and educational services to devise and implement care plans.
Provide counselling and advice to families and children, supporting them in accessing the resources and services they require.
Work within a multidisciplinary team to deliver comprehensive care and support to young people and their families.
Keep detailed records and reports, ensuring compliance with relevant legislation and policies.

What you need:

A minimum of three years post-qualifying experience, working in a permanent post within a Local Authority.
Full UK driving licence with access to your own vehicle, business cover including carrying passengers.
A current in-date DBS at the interview stage - ensuring a safe environment for our children and young people.
