What you will do:
- Conduct assessments and reviews of children and young people with disabilities to ensure their needs are being met effectively.
- Collaborate with families, healthcare professionals, and educational services to devise and implement care plans.
- Provide counselling and advice to families and children, supporting them in accessing the resources and services they require.
- Work within a multidisciplinary team to deliver comprehensive care and support to young people and their families.
- Keep detailed records and reports, ensuring compliance with relevant legislation and policies.
What you need:
- A minimum of three years post-qualifying experience, working in a permanent post within a Local Authority.
- Full UK driving licence with access to your own vehicle, business cover including carrying passengers.
- A current in-date DBS at the interview stage - ensuring a safe environment for our children and young people.
