Key Responsibilities

  • Assessment & Care Planning:

    • Conduct comprehensive assessments of adults with care and support needs, ensuring person-centred care plans that align with Worcestershire County Council’s standards.

    • Regularly review and update care plans for individuals with diverse needs, including older adults, individuals with disabilities, and those with mental health issues.

  • Safeguarding & Risk Management:

    • Lead safeguarding cases, identifying potential risks and implementing protective measures in line with the Care Act 2014.

    • Collaborate with other professionals to ensure the safety and well-being of vulnerable adults.

  • Collaboration & Advocacy:

    • Work closely with healthcare providers, therapists, and voluntary sector partners to offer holistic support.

    • Provide advocacy, information, and support to service users, carers, and families, ensuring individuals are at the centre of decision-making.

  • Compliance & Documentation:

    • Ensure compliance with the Care Act, Safeguarding Adults procedures, and Health and Social Care Act.

    • Maintain accurate and up-to-date case records in accordance with the Council’s policies and procedures.

  • Risk Assessments & Reviews:

    • Undertake thorough risk assessments to ensure that care plans and services meet individual needs while maintaining safety and independence.

    • Lead on reviews of care packages, ensuring they remain appropriate and effective.

  • Team Contribution:

    • Actively participate in team meetings, case discussions, and training sessions to promote knowledge sharing and best practices within the team.
