What you will do:
- Assess the needs of vulnerable adults and their carers, identifying innovative and personalised support methods.
- Engage with individuals having complex needs across various settings, including the Community Team for Older People and Adults with Physical or Sensory Disabilities or Hospital.
- Manage your own caseload independently, maintaining detailed case records while working within a supportive framework.
- Implement safeguarding policies effectively, ensuring the safety and well-being of adults in your care.
- Stay up to date with statutory legislation and guidance, applying this knowledge in everyday practice.
- Contribute to service development and take part in project management.
What you need:
- A qualification in social work (Dipsw, MSc, CQSW, or CSS) and HCPC registration.
- Substantial experience as a qualified social worker with a strong command of Adult Social Care eligibility criteria.
- Excellent assessment skills and a deep understanding of adult safeguarding legislation.
- The ability to manage a dynamic caseload efficiently, with strong evidence-based recording skills.
- Exceptional communication abilities and conflict resolution skills.
- A knack for working independently, demonstrating self-motivation and effective organisational skills.
- Experience in a supervisory role and proficient IT skills.
