What you will do:

- Conduct comprehensive needs assessments and deliver personalised support plans to adults and their carers following Care Act guidelines
- Develop risk assessments and implement effective intervention strategies
- Commission tailored support packages and make relevant safeguarding referrals
- Review and monitor care plans regularly to ensure quality and effectiveness
- Collaborate with partner agencies to deliver cohesive and integrated care
- Maintain meticulous case records and documentation for all assessments and interventions
- Participate in duty rotas, providing support and cover as necessary
- Attend court or tribunal hearings as required, providing expert testimony and case insights
- Support carers through thorough assessments and future mental capacity planning
- Engage in potential duties like Approved Mental Health Professional (AMHP) and Best Interests Assessor (BIA) roles along with practice education and multi-agency training initiatives
