What you will do:

Visit individuals in their home environments to conduct comprehensive assessments of their needs.
Proactively engage in the review and responsive appraisal of residents requiring or receiving one-on-one support in care homes, ensuring that care measures are both suitable and the least restrictive.
Collaborate with service providers to revise or reduce one-on-one support measures as necessary.
Support and evaluate individuals who receive Direct Payments, ensuring care packages meet their evolving requirements.
Keep detailed, up-to-date records including assessments, care plans, and submit funding requests efficiently.
