What you will do:
- Conduct comprehensive assessments of individuals within the community to identify their needs
- Develop, implement and review care plans in collaboration with other professionals
- Provide advice and support to individuals and their families, ensuring they understand their rights and entitlements
- Work effectively with partner agencies to promote the well-being of adults in the community
- Keep accurate records and maintain confidentiality in all dealings
Requirements of the Social Worker:
- A degree or equivalent qualification in social work
- Current registration with Social Work England
- Demonstrated significant frontline experience in social services within the UK as a qualified social worker
