What you will do:
- Conduct comprehensive assessments under the Care Act 2014, tailoring support plans to meet individual needs.
- Coordinate ongoing care management interventions to ensure the delivery of high-quality adult social care.
- Lead annual reviews of support plans and manage direct payment work, ensuring compliance and effectiveness.
- Facilitate the transition of young people from children's services to adult services, providing continuity and support.
- Collaborate with extra care housing support teams to enhance service delivery.
