What you will do:
- Conduct Adults with Incapacity work, primarily focusing on creating and managing effective hospital discharge plans.
- Provide expert guidance and support to service users, ensuring their needs are fully met within the community.
- Work collaboratively with multidisciplinary teams to deliver comprehensive social work services.
- Maintain accurate and up-to-date records, ensuring compliance with relevant legislation.
- Engage with service users and their families, providing them with the necessary support and information to navigate the care system.
Essentials:
- A degree or equivalent qualification in social work
- Current registration with Social Work England
- Significant frontline experience in social services within the UK
