This role sits within a collaborative team dedicated to exceptional social work. Core functions include assessments, support planning, long-term support arrangements, and reviews for individuals in the community who have complex needs. You will carry out Mental Capacity Assessments and deliver a social work service to your allocated caseload, ensuring performance management, quality assurance, and budgetary frameworks are met to provide supreme service standards.

What you will do:

- Conduct comprehensive assessments and develop detailed support plans for individuals with learning disabilities.
- Implement long-term support arrangements and regularly review and adjust plans to meet the evolving needs of your clients.
- Carry out Mental Capacity Assessments and offer guidance in decision-making processes.
- Deliver personalised support and brokerage services for adults, carers, and families, ensuring accessible, high-quality services.
