What you will do:

  • Manage an identified caseload with responsibility for specialist assessments in collaboration with a Band 6 Nurse.
  • Undertake patient assessments and formulate person-centred care plans and risk assessments for service users.
  • Coordinate care plans and deliver nursing interventions to individuals facing mental health difficulties, particularly related to dementia and memory challenges.
  • Participate in quality assurance, auditing, and clinical governance activities to ensure service excellence.
  • Engage with multi-disciplinary teams and external agencies, including GPs and support organisations, to provide comprehensive care.
  • Administer medication safely and effectively in accordance with protocols.
  • Conduct formal and informal training sessions for students, staff, carers, and service users.
  • Maintain accurate data collection and reporting, ensuring confidentiality and adherence to Trust policies.
