What you will do:
- Provide expert health advice and support to children and young people living within the Home.
- Carry out comprehensive health assessments, known as CHATs, and develop personal health plans for the residents.
- Deliver targeted health promotion and training sessions for staff, children, and their families.
- Collaborate with other health and social care professionals to ensure young people have access to universal and specialist health services.
- Work proactively to improve health outcomes through personalised packages of care, supporting young people in their transition back to the community.
- Promote a strength-based and trauma-informed approach to care in line with the ethos of the Health and Wellbeing Team.
- Contribute to the development of an inclusive, innovative service shaped by listening to the needs and aspirations of young people.
