About the Role:

Based in South Gloucestershire, you’ll play a vital role in promoting independence and delivering person-centred social care in line with the Care Act 2014. You’ll conduct assessments, develop care plans, and coordinate safeguarding interventions to ensure adults receive the right support at the right time. Collaboration with health, housing, and voluntary partners will be central to achieving positive outcomes.

What You Will Do:

• Conduct strength-based assessments that identify personal strengths, networks, and resources.

• Develop and review person-centred care plans promoting independence and wellbeing.

• Undertake safeguarding enquiries (Section 42) and manage risk in line with statutory frameworks.

• Provide case management for adults with complex needs, including hospital discharges and community-based support.

• Work collaboratively with NHS partners, housing teams, and voluntary organisations for integrated service delivery.

• Maintain accurate, timely case records on Mosaic and adhere to professional standards.

• Participate in team meetings, reflective supervision, and contribute to service improvement initiatives.

• Complete Carer’s Assessments and facilitate access to appropriate support services.

• Uphold Making Safeguarding Personal principles in all aspects of practice.

About You:

You are a qualified and registered Social Worker (SWE) with solid knowledge of the Care Act 2014, Mental Capacity Act 2005, and related legislation. You excel in assessment, analytical, and report-writing skills, with a passion for delivering strength-based, person-centred, and outcome-focused practice. Experience in community-based adult social care will be highly beneficial.
