Seven Resourcing is seeking an experienced and motivated Reablement Assessor to join a hospital-based reablement service in Scotland. This full-time position (37 hours per week) is available for an immediate start.

We welcome applications from Occupational Therapists, Social Workers, or Physiotherapists with strong experience in reablement, discharge-to-assess (D2A) practice, community rehabilitation, and functional assessment.

Responsibilities

• Complete person-centred functional assessments to determine individual needs and goals
• Support rapid hospital discharge and safe transition into community settings
• Develop and review reablement plans, monitoring outcomes and adjusting support as required
• Promote independence, rehabilitation, and recovery in line with strengths-based practice
• Work collaboratively across multi-disciplinary teams
• Maintain accurate documentation and case records

Essential Requirements

• Qualified Occupational Therapist, Social Worker, or Physiotherapist with valid HCPC/SSSC registration
• Proven background in reablement, D2A, intermediate care, community rehab, or preventative care services
• Strong knowledge of person-centred and strengths-based approaches
• Understanding of local authority practice, safeguarding legislation, and adult social care frameworks
• Strong assessment, communication, and report-writing skills
• Able to work effectively within multi-disciplinary teams
