What You Will Do

Manage a diverse caseload, supported by a dedicated team, to deliver high-quality adult social care services.
Conduct thorough assessments and develop personalised support plans tailored to individual needs.
Prepare and contribute to court hearings, including applications to the Court of Protection, with detailed assessments and reports.
Engage effectively in challenging situations, representing the department in multi-disciplinary meetings and forums.
Deliver face-to-face interventions with service users, coordinating with carers, advocates, and partner agencies.
Monitor and review existing support plans to ensure outcomes meet assessed needs.
Participate in financial planning to ensure efficient and sustainable service delivery.
Uphold safeguarding responsibilities, fostering an inclusive, professional, and supportive environment.
