What You Will Do:

  • Undertake strengths-based assessments and develop personalised care and support plans in line with statutory responsibilities.

  • Work collaboratively with adults, carers, and families to identify outcomes, strengths, and needs, ensuring appropriate support is in place.

  • Manage a varied caseload, including complex cases requiring joint work with health and social care professionals.

  • Conduct safeguarding enquiries in accordance with Section 42 of the Care Act 2014.

  • Apply the Mental Capacity Act 2005 to support individuals in making informed decisions.

  • Liaise with healthcare providers, voluntary agencies, and care partners to promote integrated care.

  • Monitor and review care plans to ensure outcomes are being met effectively.

  • Maintain accurate case records following professional standards and council policies.

  • Participate in team meetings, supervision, and service development initiatives.

  • Promote independence and wellbeing, supporting individuals to access local community and preventative resources.


Qualifications and Experience:

Essential:

  • Degree in Social Work (CQSW, DipSW, BA, or MA).

  • Current registration with Social Work England.

  • Minimum 2 years’ post-qualified experience in adult social care.

  • Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Human Rights Act 1998, and safeguarding frameworks.

  • Experience supporting adults with physical disabilities, mental health needs, or older people.

Desirable:

  • Prior experience within a local authority community social work team.

  • Knowledge of Liquidlogic or similar case management systems.

  • Excellent assessment, analytical, and problem-solving skills.

  • Strong communication and interpersonal abilities.

  • Ability to build effective relationships with service users, carers, and professionals.

  • Highly organised, able to prioritise and manage caseloads effectively.
