Conduct comprehensive assessments in line with the Care Act 2014 and the Mental Capacity Act 2005, ensuring the best outcomes for adults in the community.
Manage safeguarding concerns and enquiries in accordance with Section 42 of the Care Act, ensuring the safety and well-being of vulnerable individuals.
Navigate the CHC processes, including completing checklists and participating in MDT/Decision Support Tool meetings.
Collaborate effectively with health and social care colleagues, contributing to a dynamic and balanced team.
Utilise your initiative to drive positive outcomes and advocate for adults within the North Community Mental Health Team.
