An exciting opportunity has arisen for a Qualified Adult Social Worker to join the Learning Disabilities and Autism Team within Adult Social Care in Telford. What You Will Do:

  • Carry out Care Act assessments and reviews for adults with learning disabilities and autism.

  • Work strategically to maximise healthcare funding and resources for service users.

  • Develop and oversee personalised care plans, working closely with individuals and their families.

  • Provide advocacy and support to ensure service users' rights, choices, and wellbeing are always prioritised.

  • Maintain accurate, confidential case records and documentation.

  • Collaborate with multi-disciplinary teams to coordinate holistic, effective support.

  • Participate in supervision, team meetings, and case discussions to drive continuous improvement.
