What you will do:

- Participate as a key member of the Intermediate Care Team, delivering high-quality physiotherapy services.
- Work effectively across various pathways: reablement, rapid response, and rehabilitation, aiming to optimise patient outcomes at each stage.
- Conduct comprehensive assessments of patients in community settings, developing and implementing personalised treatment plans.
- Collaborate with a multidisciplinary team to ensure holistic care for patients, sharing expertise and insights.
- Provide education and support to patients and families to ensure the continuation of care within their homes.
- Maintain accurate and up-to-date patient records, ensuring compliance with HCPC standards.

Requirements:

To thrive in this role, you need to be HCPC registered, with some post-graduate experience due to the complexity of the caseload. Prior experience working in the community and within a multidisciplinary team is highly desirable. A full driving licence is essential for this role as you will need to travel between various locations in Walsall.
