Key Responsibilities:

• Deliver high-quality physiotherapy across all Intermediate Care pathways, including reablement, rapid response, and rehabilitation.

• Work collaboratively within a multidisciplinary team to achieve shared goals and improved patient outcomes.

• Conduct comprehensive assessments and develop personalised care plans for patients requiring intermediate care.

• Use your community care experience to deliver effective rehabilitation programmes that promote independence.

• Maintain professional standards and engage in ongoing learning and development opportunities.

Requirements:

• HCPC registration.

• Full driving licence and ability to travel, due to the community-focused nature of the role.

• Preferably post-graduate experience within MDT and rehabilitation settings to manage complex caseloads effectively.
