What You Will Do:

• Conduct holistic assessments for patients recently discharged from hospital, promoting safe and effective transitions back into the community.

• Develop and deliver tailored rehabilitation plans that address individual patient needs and optimise recovery outcomes.

• Collaborate closely with multidisciplinary colleagues, ensuring integrated and person-centred care.

• Promote independence and mobility, empowering patients to achieve their functional goals.

• Maintain accurate and detailed documentation in line with NHS and professional standards.

This role requires registration with the Health and Care Professions Council (HCPC) and a valid UK driving licence to undertake community visits efficiently.
