What You Will Do:

• Conduct comprehensive occupational therapy assessments and develop personalised reablement plans in service users’ homes.

• Deliver tailored interventions to promote independence, wellbeing, and recovery in alignment with individual goals.

• Collaborate closely with the multi-disciplinary team, including physiotherapists, social workers, and nurses, to provide holistic support.

• Assess the need for aids, adaptations, and specialist equipment, ensuring that all recommendations enhance safety and independence.

• Carry out manual handling assessments, advising on appropriate equipment and safe handling techniques.

• Maintain accurate and timely documentation, utilising electronic case recording systems.

• Offer guidance and mentorship to students, assistants, and junior colleagues, fostering continuous professional development within the team.

About You:

• HCPC-registered and qualified Occupational Therapist.

• Experienced in community-based or integrated care environments.

• Passionate about person-centred, strengths-based practice.

• Confident communicator and effective collaborator in multidisciplinary settings.

• Organised, proactive, and adaptable to fast-paced work environments.

• A full UK driving licence is essential for this role to provide effective outreach across the Southwark area.
