What You Will Do:

Conduct comprehensive occupational therapy assessments and develop personalised reablement plans in service users’ homes.

Deliver tailored interventions to promote independence, wellbeing, and recovery in alignment with individual goals.

Collaborate closely with the multi-disciplinary team, including physiotherapists, social workers, and nurses, to provide holistic support.

Assess the need for aids, adaptations, and specialist equipment, ensuring that all recommendations enhance safety and independence.

Carry out manual handling assessments, advising on appropriate equipment and safe handling techniques.

Maintain accurate and timely documentation, utilising electronic case recording systems.

Offer guidance and mentorship to students, assistants, and junior colleagues, fostering continuous professional development within the team.
About You:

HCPC-registered and qualified Occupational Therapist.

Experienced in community-based or integrated care environments.

Passionate about person-centred, strengths-based practice.

Confident communicator and effective collaborator in multidisciplinary settings.

Organised, proactive, and adaptable to fast-paced work environments.

A full UK driving licence is essential for this role to provide effective outreach across the Southwark area.
