What You Will Do:
- 
Lead and coordinate tailored rehabilitation programmes alongside reablement officers to support clients in achieving their goals.
 - 
Manage a varied caseload that includes hospital discharges and community-based referrals, providing up to six weeks of focused rehabilitation.
 - 
Conduct comprehensive assessments and interventions aimed at promoting independence and preventing long-term care needs.
 - 
Utilise your clinical expertise in physical rehabilitation and case management within a dynamic, fast-paced community setting.
 - 
Engage in multi-disciplinary teamwork to ensure holistic, patient-centred care and optimal outcomes.
 - 
Travel within the community to deliver responsive and effective care directly to clients’ homes.
 
Requirements:
- 
HCPC registration as an Occupational Therapist.
 - 
Minimum of two years’ post-qualification experience within adult social care, reablement, or rehabilitation.
 - 
Strong assessment, communication, and documentation skills.
 - 
A valid driving licence and access to a vehicle are highly desirable.
