This role plays an integral part in transitioning patients safely from hospital to home, ensuring seamless care. What you will do:

  • Participate in hospital discharge planning by conducting social work assessments to ensure swift and effective transitions in line with national and local guidelines.
  • Perform strengths-based assessments under the Care Act 2014, focusing on individuals’ capabilities, resources, and desired outcomes.
  • Carry out thorough risk assessments to promote safe discharges and community safety, escalating concerns when needed.
  • Develop, implement, and review care and support plans, coordinating a variety of services including reablement and long-term care.
  • Engage with NHS colleagues, reablement teams, housing providers, and voluntary organisations to maintain seamless care transitions.
  • Address safeguarding issues adhering to Dorset Council’s policies to protect vulnerable adults.
  • Undertake mental capacity assessments, applying the Mental Capacity Act 2005 principles to make informed Best Interests decisions.
  • Effectively manage a busy caseload, prioritising tasks to meet urgent discharge demands.


About you:

You must possess a recognised Social Work qualification, be registered with Social Work England, and ideally have experience in hospital discharge settings. You should be prepared to manage complex cases and adapt to a fast-paced work environment. Enhanced DBS Clearance (Children’s & Adults) will be arranged if you join us, and you must have the right to work in the UK.
