What you will do:

- Assess, diagnose, and manage patients with acute or long-term conditions; take clinical histories, perform examinations, and interpret results.
- Prescribe medications safely and appropriately within your scope of practice, adhering to national guidelines such as those from the National Institute for Health and Care Excellence.
- Monitor treatment effectiveness, review medications, manage side effects, and optimise medicines use.
- Educate patients about their conditions and medications, support treatment adherence, and provide lifestyle advice.
- Maintain accurate clinical documentation and records of assessments, prescribing decisions, and patient outcomes.
- Collaborate with multidisciplinary teams including GPs, pharmacists, and specialists, ensuring that patients receive the comprehensive care they need.
- Comply with prescribing regulations and professional standards set by bodies such as the Nursing and Midwifery Council and the General Pharmaceutical Council, and participate in audits and continuous professional development.
