What you will do:

- Deliver occupational therapy interventions in patients' homes and community settings, ensuring individuals can maintain their independence.
- Conduct comprehensive assessments and create bespoke intervention plans for those with long-term complex needs.
- Support admission avoidance through prompt therapeutic input, enabling individuals to remain within their communities.
- Recommend, arrange, and manage assistive equipment and home adaptations, including processing Disabled Facilities Grants (DFGs).
- Conduct housing assessments and contribute to manual handling and reablement plans.
- Participate in a weekly Duty rota to respond to queries and undertake urgent visits efficiently.
- Act as a Trusted Assessor to set up and review care packages based on assessed needs.
- Efficiently manage a varied caseload, demonstrating strong organisational and clinical reasoning skills.

Requirements:
You must be HCPC registered, a skilled car driver and flexible enough to handle multiple cases effectively and creatively.
