What you will do:

- Communicate with patients and families, providing education and support to navigate the discharge process.
- Document all patient assessments, plans, and progress comprehensively and accurately.

Requirements:
- HCPC Registered Physiotherapist ready to hit the ground running.
- Degree or equivalent qualification in Physiotherapy, ensuring you have the expertise needed.
- Proven experience in the UK, NHS, or hospital settings as a Physiotherapist is essential for this role.
