What you will do:
- Conduct timely physiotherapy assessments on inpatient or reablement wards to assess discharge readiness and determine necessary physiotherapeutic interventions.
- Engage in parallel planning to ensure seamless care delivery for patients.
- Develop and monitor individualised exercise and rehabilitation plans tailored to each patient's unique needs.
- Liaise effectively with hospital, community, and multidisciplinary teams to coordinate and optimise patient care.
- Support the timely flow and discharge of patients, ensuring a smooth transition from hospital to home or other care settings.

Requirements of the Physiotherapist:
- You must be a qualified physiotherapist with a degree or equivalent in Physiotherapy.
- Recent UK experience in specialized physiotherapy is required.
- HCPC registration as a physiotherapist in the UK is a must.
- NHS experience is advantageous but not essential.
