This is an exciting ongoing opportunity for a Community Physiotherapist to join a thriving Community Social Services team in Bolton. You’ll be part of an Independent Living Service, working closely alongside Occupational Therapists and Community Officers. The role spans both Discharge to Assess at Home (DTAH) and Independent Living Services (ILS), with close collaboration across hospitals, Bolton at Home, and key service partners.

What you will do:

• Assess and identify physiotherapy needs of service users within the community.

• Develop and implement personalised rehabilitation and treatment plans.

• Work collaboratively within a multidisciplinary team, including Occupational Therapists and Community Officers.

• Engage with service users and their families to support holistic, person-centred care planning.

• Support safe and timely hospital discharges through the Discharge to Assess at Home (DTAH) pathway.

Requirements:

• HCPC registration as a Physiotherapist.

• A valid UK driving licence, with the ability to travel across Bolton and surrounding areas for community visits.
