What you will do:

• Provide expert clinical ward-based pharmacy services to support patient care.
• Collaborate with healthcare professionals to ensure optimal medication management and safety.
• Supervise and mentor junior pharmacy staff, enhancing their skills and knowledge.
• Conduct regular medication reviews and provide patients with advice to promote effective treatment regimes.
• Ensure compliance with hospital policies and procedures, maintaining a high standard of pharmaceutical care.
• Participate actively in multidisciplinary team meetings, contributing to patient-centred care planning.

Requirements:

• Must be registered with the GPhC.
• A minimum of 2 years of experience in hospital pharmacy is necessary, with a specialism in clinical pharmacy preferred.
• A post-graduate clinical pharmacy qualification is highly desirable.
• Being a car driver is preferable to facilitate travel to multiple sites.
