The role is contracted for 3 months and includes working hours of 37.5 per week with shifts from 09:00-17:00 and 11:00-19:00, which may later extend from 08:00-20:00. What you will do

• Provide physiotherapy assessments and personalised rehabilitation plans.
• Deliver tailored treatment programmes and set achievable goals.
• Work independently as a competent practitioner.
• Support admission avoidance assessments.
• Facilitate early supported discharges from acute hospital services.
• Engage in inpatient rehabilitation pathways.
• Perform comprehensive assessments using your clinical reasoning skills.
• Manage clinical risk judiciously within your therapy caseload.
• Collaborate effectively with multidisciplinary teams to plan patient discharges.
• Maintain precise electronic clinical records.
• Participate actively in developing integrated care pathways.
• Supervise designated team members as needed.

Requirements

• Degree or Diploma in Physiotherapy.
• HCPC registration.
• Proven Band 6 Physiotherapy experience.
• Experience in community and inpatient rehabilitation.
• Ability to support early discharge pathways.
• Strong autonomous working skills.
• Excellent clinical assessment and reasoning capabilities.
• Experience within multidisciplinary teams.
• A full UK driving licence and access to a vehicle for work purposes.

Desirable Requirements

• CSP membership.
• Experience supervising staff or students.
• Audit and service improvement experience.
• Community rehabilitation experience.
