What you will do

• Independently manage a caseload of complex older adults.

• Assess mobility, transfers, and falls risk.

• Evaluate rehabilitation potential and functional ability.

• Design and implement short-term rehabilitation programmes.

• Facilitate safe and timely discharge home.

• Lead discharge planning from admission through community transition.

• Coordinate onward referrals to community services.

• Collaborate effectively with multidisciplinary teams.

• Support and mentor junior staff and students.

• Keep accurate clinical records and documentation.

Requirements

• Degree or Diploma in Physiotherapy.

• HCPC registration.

• Experience as a Band 6 Physiotherapist.

• Expertise in working with older adults.

• Rehabilitation experience, including mobility and falls risk assessment.

• Proficiency in discharge planning and independent caseload management.

• Full UK driving licence and the ability to travel between Newmarket and Sudbury sites.

Desirable Requirements

• Experience in community rehabilitation and intermediate care.

• Background supporting junior staff and students.

• NHS rehabilitation experience.
