What you will do:

- Conduct timely physiotherapy assessments on DMOPs and Medicine wards to determine patient discharge readiness and required interventions.
- Develop, implement, and monitor individualised exercise and rehabilitation plans paralleling patient needs and progress.
- Liaise effectively with hospital and community teams, enhancing coordinated care and ensuring timely patient flow and discharge.
- Offer support with educational resources for patients and their families, promoting health awareness and engagement.
