What you will do:

- Deliver intensive rehabilitation to maximise independence for adults, particularly older adults, with complex needs.
- Coordinate with a multidisciplinary team to prevent unnecessary hospital admissions and facilitate early discharge.
- Develop and implement therapy plans focused on enabling patients to safely remain or return home.
- Perform assessments and follow-up evaluations to ensure effective patient outcomes.

Requirements:

- HCPC Registered Occupational Therapist with a minimum of 2 years of experience.
- Driving preferred but not essential, facilitating home visits and community-based therapy.
