What you will do:

- Carry a caseload of children with life-limiting conditions, ensuring family-centred care.
- Develop and maintain high standards of nursing care within a child's home setting.
- Act as a key worker, training carers and supporting families for sustained independence.
- Liaise with agencies, working collaboratively within a multi-disciplinary team.
- Embrace clinical governance, auditing, and evidence-based practice to enhance care.
- Provide community nursing support, with flexibility to meet families' needs over a 37.5-hour week.
- Support early discharges and reduce hospital admissions by providing essential home care.
- Engage in policy development and ensure adherence to professional standards.
