We are seeking a committed Band 6 Keyworker to join our Mental Health Team based in Aylesbury, UK. This role offers a fantastic opportunity to work within an innovative healthcare setting known for its supportive and collaborative environment. You'll be working full-time, 37.5 hours a week, Monday to Friday from 09:00 to 17:00, making a real difference in the lives of individuals experiencing mental health challenges. If you have experience assisting those with psychosis, managing risk, and are adept with documentation and duty tasks, this might just be the exciting role you’ve been waiting for. Familiarity with Trust IT systems will be a bonus.

What you will do:

Hold a caseload and provide tailored support to individuals experiencing mental health issues, ensuring high standards of care and support.
Conduct assessments and develop comprehensive care plans that meet the individual needs of service users.
Manage and assess risks, providing interventions that promote recovery and personalisation.
Liaise with multidisciplinary teams to deliver cohesive and collaborative care support for each service user.
Maintain accurate and up-to-date documentation in line with legal requirements and Trust policies.
Attend and contribute to clinical meetings, supervision, and group support sessions.
Utilise Trust IT systems to enhance communication and streamline service delivery.
