This is an exciting opportunity for a Band 6 Community Psychiatric Nurse to join a well-established Community Psychiatry service in Loughton. Working 37.5 hours per week, you will play a key role in delivering high-quality, person-centred mental health care within the community. What you will do:

Manage and coordinate the care of service users within the community setting
-
Complete comprehensive mental health assessments and develop personalised care plans

Undertake risk assessments and implement strategies to maintain service user safety

Maintain accurate and timely clinical documentation in line with Trust policies

Attend and actively contribute to multidisciplinary team meetings

Administer medication, monitor side effects, and promote adherence to treatment plans

Provide mental health education and support to service users and their carers
-
Contribute to audits, service development, and evidence-based practice initiatives
Requirements:
-
Registered Mental Health Nurse with current NMC registration
-
Experience working within community mental health services
-
Strong assessment, risk management, and care coordination skills

Ability to work autonomously while contributing effectively to a wider team

Commitment to delivering high-quality, recovery-focused care
