What you will do:
- Perform independent assessments of service user care needs, which may be highly complex, to develop, implement, and evaluate tailored care programmes.
- Ensure that care plans are co-produced with service users and carers, maintaining relevance and accessibility for the multidisciplinary team.
- Contribute to the Community Mental Health Service using effective evidence-based practices aligned with national standards for clinical practice.
- Act as a keyworker for service users with complex needs, sharing sensitive information appropriately and in line with the Consent to Share Information Policy.
- Provide structured therapeutic interventions through individual or group sessions, adapting to the specific complexities of each client.
- Plan and deliver all aspects of patient care according to the NMC Code of Practice or relevant professional standards.
- Drive continual improvements in clinical care standards within the clinical governance framework to ensure the safest and highest quality of care is delivered.
