What you will do:
- Conduct assessments of individuals with substance misuse issues, providing a solid foundation for tailored recovery plans.
- Develop and review personalised recovery plans, ensuring they are adapted to meet the unique needs of each service user.
- Provide one-on-one recovery support and harm reduction advice to empower individuals on their journey to recovery.
- Offer guidance on life skills, relapse prevention, and recovery goals, contributing positively to the lives of those you support.
- Build trusting relationships and engage resistant clients, serving as a pivotal advocate in accessing necessary support services.
- Work closely with other professionals across sectors such as healthcare and social work, fostering a multidisciplinary approach to care.
- Facilitate group sessions for relapse prevention and life skills, and educate clients on substance misuse and the recovery process.
- Regularly monitor service users’ progress, adjusting care plans as necessary and maintaining accurate case records.
- Identify and manage safeguarding risks, ensuring the safety and well-being of service users throughout their recovery journey.
- Stay updated on best practices in substance misuse recovery and participate in training to enhance your professional skills.
