Advanced Social Work Practitioner – Friends & Family Team

Pay Rate: £40 per hour
Contract Length: 3 months (Full-time, Locum)

We have an exciting opportunity for an Advanced Social Work Practitioner to join the Friends & Family Team in Liverpool on a full-time locum basis for an initial three-month period. Offering an attractive rate of £40 per hour, this role gives you the chance to make a real difference in supporting care planning and permanence for young people transitioning from foster care.

The Staying Put Liaison Worker role involves supporting young people from the age of 16 and providing ongoing guidance to former foster carers post-18, ensuring stable and positive outcomes. Your expertise will play a key role in driving quality, performance, and best practice within the Fostering Service.

What you will do:

Monitor care plan progress and share relevant information with foster carers to achieve sustained Staying Put arrangements
Provide ongoing support and guidance to former foster carers post-18, ensuring stability and security for young people
Conduct regular 12-weekly visits to support Staying Put hosts, maintaining continuity and stability for young adults
Drive quality and performance within the Fostering Service by offering consistent oversight and scrutiny of care planning
Identify and challenge instances of drift or delay, ensuring timely and effective interventions
