What You Will Do:

• Lead and mentor a team of Community Care Officers, promoting effective, high-quality practice.

• Conduct urgent social care assessments and interventions to help individuals live safely and independently.

• Drive the development of innovative, community-based care solutions by collaborating with health and voluntary sector partners.

• Fulfil statutory duties, including safeguarding, assessment, care management, and supervision.

• Build and maintain multi-agency partnerships, ensuring culturally responsive and inclusive services.

• Support adults at risk of hospital admission through early intervention and collaborative care planning.

• Provide guidance and support to unpaid carers, enhancing resilience and community engagement.

About the Role:
