What You Will Do:

Lead and mentor a team of Community Care Officers, promoting effective, high-quality practice.

Conduct urgent social care assessments and interventions to help individuals live safely and independently.

Drive the development of innovative, community-based care solutions by collaborating with health and voluntary sector partners.

Fulfil statutory duties, including safeguarding, assessment, care management, and supervision.

Build and maintain multi-agency partnerships, ensuring culturally responsive and inclusive services.

Support adults at risk of hospital admission through early intervention and collaborative care planning.

Provide guidance and support to unpaid carers, enhancing resilience and community engagement.
About the Role:
