Key Responsibilities:

• Provide expert clinical assessment, diagnosis, and management for patients in community settings.

• Exercise advanced decision-making and clinical judgment to deliver safe, effective care.

• Collaborate within a multi-disciplinary team to ensure holistic and high-quality patient outcomes.

• Utilise your V300 Independent Prescriber qualification to prescribe and manage treatments confidently.

• Support the design and implementation of personalised care plans.

Requirements:

• Car driver – essential for community-based visits.

• Proven community experience, confident working independently in home and local settings.

• V300 qualification (Independent Prescriber).

• Fully trained Advanced Clinical Practitioner with completed ACP training and documented competencies.
