What you will do

Conduct comprehensive assessments, reviews, and support planning in line with the Care Act 2014.
Manage a varied caseload involving adults with complex needs, including physical disabilities, mental health conditions, and learning disabilities.
Undertake safeguarding enquiries in accordance with Section 42 guidelines.
Carry out Mental Capacity Act assessments and make Best Interest decisions.
Work collaboratively with families, health professionals, and partner agencies to deliver holistic care planning.
Support hospital discharge processes and coordinate community‑based care services.
Maintain accurate, detailed case records while promoting independence and wellbeing.

Requirements

Essential:

Qualified Social Worker with current Social Work England registration.
Experience in Adult Social Care with strong knowledge of the Care Act 2014 and safeguarding procedures.
Ability to manage complex caseloads independently and effectively.

Desirable:

Experience working within a UK local authority.
Knowledge of Continuing Healthcare (CHC) and familiarity with case management systems such as Liquid Logic.
