What You Will Do

  • Conduct strengths-based assessments in accordance with the Care Act 2014 to identify individual needs and outcomes.

  • Develop and manage personalised care and support plans that promote independence and wellbeing.

  • Undertake Mental Capacity Assessments and facilitate best-interest decisions in line with the Mental Capacity Act 2005.

  • Support service users and carers through complex decision-making processes.

  • Address safeguarding concerns and implement appropriate interventions to protect adults at risk of abuse or neglect.

  • Work collaboratively with health services, housing providers, and third-sector organisations to ensure coordinated support.

  • Participate in multidisciplinary meetings and contribute to joint care planning and decision-making.

  • Support effective transitions between hospital and community-based services.

  • Manage a varied caseload while prioritising risk, complexity, and service user outcomes.

  • Maintain accurate and up-to-date case records in line with council policies and professional standards.

  • Prepare detailed reports to support assessments, reviews, and case conferences.

Requirements

  • Qualified Social Worker with a recognised qualification such as BA/MA Social Work, DipSW, or equivalent.

  • Current Social Work England registration.

  • Experience working within Adult Social Care in a statutory setting.

  • Strong knowledge of the Care Act 2014, Mental Capacity Act 2005, and safeguarding procedures.

  • Ability to manage complex cases within a multidisciplinary environment.

  • Excellent communication, assessment, and decision-making skills.

Desirable Criteria

  • Experience working with older adults, physical disabilities, or mental health services.

  • Familiarity with Continuing Healthcare processes and hospital discharge pathways.

  • Ability to work effectively within a fast-paced local authority environment.
