What you will do:
- Contribute to the operational delivery of an effective Adult Social Care Service focused on safeguarding, support planning, and promoting the wellbeing of adults with care and support needs.
- Identify individual needs through comprehensive assessments and plan co-designed individual support plans.
- Engage in regular monitoring and reviews of support plans to ensure they meet the set objectives and adapt them as necessary.
- Manage your caseload independently while upholding the reputation of the Department and Council, ensuring compliance with national and service procedures and guidance.

Requirements of the Social Worker:
- Possess a degree or equivalent in social work with current Social Work England registration.
- Have significant frontline experience in Social Services within the UK working as a qualified Social Worker.
