What you will do

• Mental Health Assessments: Conduct comprehensive Mental Health assessments to identify needs, risks, and appropriate intervention pathways.
• Front Door Screening: Support the Contact Team by ensuring referrals are assessed promptly and effectively.
• Decision Making: Provide professional analysis and recommendations to ensure individuals receive appropriate support and services.
• Crisis Intervention: Respond to urgent situations and provide advice and intervention where required.
• Multi-Agency Working: Collaborate with healthcare professionals, community services, mental health providers, and partner agencies.
• Care Planning: Contribute to short-term intervention planning and recommendations for ongoing support where appropriate.
• Record Keeping: Maintain accurate, timely, and comprehensive case records in accordance with statutory and organisational requirements.
• Professional Practice: Ensure all work is completed in line with relevant legislation, policy, and professional standards.

Qualifications / Requirements

• Social Work Qualification: Degree in Social Work or equivalent recognised qualification.
• Registration: Current Social Work England registration.
• Experience: Previous experience within Adult Social Care and Mental Health Services.
• Mental Health Knowledge: Strong understanding of mental health legislation, safeguarding procedures, and strengths-based practice.
• Assessment Skills: Proven experience undertaking Mental Health assessments and risk assessments.
• Communication Skills: Excellent verbal, written, and interpersonal communication abilities.
• Partnership Working: Ability to work effectively with multidisciplinary teams and external agencies.
• Organisational Skills: Ability to prioritise workloads and make timely decisions within a fast-paced environment.
