Explore this exciting opportunity as a full-time Adults Senior Social Worker specialising in Hospital Discharge in the vibrant area of Kingston. With an ongoing contract and an attractive rate of £36 per hour, you’ll be at the heart of a dynamic hospital team. Here, you will facilitate discharges using the Discharge to Assess (D2A) model from Kingston Hospital and other Out of Borough hospitals. This role offers a great mix of local engagement and broader responsibilities, plus the chance to be based at Kingston Hospital one day a week, with travel flexibility when required.

What you will do:
- Facilitate timely and efficient hospital discharges using the D2A model.
- Conduct thorough assessments ensuring all discharges are safe and appropriate.
- Liaise with multi-disciplinary teams to ensure patient needs are met effectively.
- Maintain accurate records, utilising LAS/IAS systems, with sound IT skills.
- Travel to Kingston and other necessary locations to perform essential job duties and complete assessments.
Requirements:
- A degree or equivalent qualification in social work.
- Current registration with Social Work England.
- Significant frontline experience in social services within the UK, particularly in social work roles.
- Recent sound knowledge and experience of hospital discharge processes, with advantageous knowledge of LAS/IAS and Google platforms.
