Role Overview

An exciting opportunity has arisen for an experienced Adults Social Worker to join the Older People & Physical Disabilities (OP & PD) Team within Adult Social Care at Reading Borough Council.

In this role, you will support adults with age-related needs, long-term conditions, and physical disabilities, helping them maintain independence and improve their wellbeing. You will undertake strength-based assessments, develop person-centred care plans, and work closely with multidisciplinary partners to ensure individuals receive the right support and services.

Key Responsibilities / What You Will Do

  • Conduct comprehensive needs assessments for older people and adults with physical disabilities in line with the Care Act 2014.

  • Develop and implement person-centred care and support plans that promote independence and improve quality of life.

  • Manage a diverse and complex caseload, ensuring timely completion of assessments, reviews, and required interventions.

  • Undertake safeguarding enquiries and risk assessments to protect vulnerable adults from harm.

  • Work collaboratively with health professionals, occupational therapists, housing teams, and community services to deliver holistic care.

  • Support individuals and families in accessing community resources, support services, and managing direct payments.

  • Contribute to hospital discharge planning to ensure safe and effective transitions back into the community.

  • Maintain accurate and up-to-date case records in line with council policies and statutory responsibilities.


Requirements

Essential

  • Degree or Diploma in Social Work.

  • Registration with Social Work England.

  • Experience working within Adult Social Care services.

  • Strong knowledge of the Care Act 2014, safeguarding procedures, and strength-based practice.

  • Ability to manage complex caseloads and work effectively within multidisciplinary teams.

Desirable

  • Experience working within Older People or Physical Disabilities teams.

  • Previous experience within a local authority setting.
