What you will do:
- Assess the functional needs of Service Users in various environments, coordinating service provision, rehabilitative advice, and information.
- Facilitate services as per The Care Act 2014 and The Children and Families Act ensuring a supportive approach for service users and carers.
- Conduct assessments of functional abilities utilising appropriate tools and methods, advocating for independence, wellbeing, and choice.
- Identify and refer complex or safeguarding issues to the appropriate social work team.
- Manage a diverse caseload where there is potential risk of harm, drawing on your expertise and experience.
- Lead Occupational Therapy assessments suggesting suitable interventions to safeguard service users' independence and safety.
- Conduct assessments for carers, assessing and managing associated risks comprehensively.
- Organise provision, installation, and demonstration of necessary equipment and adaptations, ensuring you stay abreast of new methods and services.
- Collaborate with other professionals and services to promote cohesive joint working initiatives.
Experience required:
Registered with the HCPC and a minimum of 2 years' experience in a relevant role are essential. Holding a driving licence is preferable due to the nature of community work.
