What you will do:
- Coordinate and deliver comprehensive care to individuals with complex mental health conditions within the community.
- Conduct assessments and liaise with multidisciplinary teams to develop effective care plans.
- Provide expert guidance and support to service users and their families, empowering them to achieve their recovery goals.
- Engage in collaborative working with other healthcare professionals and services, enhancing the quality of care provided.
- Stay updated with evidence-based interventions, ensuring the delivery of the highest standard of mental health care.
What you'll need:
- Be a Registered Nurse (Mental Health) or hold professional registration in Social Work or Occupational Therapy.
- Demonstrate evidence of ongoing professional development to maintain up-to-date knowledge in mental health.
- Have experience in community mental health, such as involvement with Home Treatment Teams or Community Mental Health Teams, or experience within acute inpatient settings.
- Possess knowledge of evidence-based interventions for complex mental health conditions, enabling you to provide high-quality care.
